I'm facing the same issue on 4.1.1 and 4.1.2 both, no anything special found on error log
No any CGI application, no any Output/Inout BW limitation, just simple lsphp5 processes are running
AIO, epoll and noatime (fstab) are used on the box
here is top head output:

Swap: 2096472k total, 0k used, 2096472k free, 30505516k cached
look at sys CPU usage, lshttpd is eating all CPU power
I'll switch to 4.0 and seeing, but any idea on this?
