LSWS defacing all sites with log entries

I thought I made a post about this before but cannot find it. This is a very serious bug and I have been getting many complaints about it. At first it seemed to happen only under high load or if a site on the server was being ddosed or something but now it seems to be happening randomly for no reason at all. One of my clients says he can make this happen by merely surfing his site.

PLease get us an update to fix this issue as soon as possible
