It is limited by the maximum size of each XML configuration value,
Right now the limit is about 1MB, so it should be able to hold 58K IPs. How to maintain this list is another story.

can we put comment lines in the denied list? like a reminder or notes

i.e.

213.x.x.x.x #spammer 12/22/08
67.x.x.x. #note here

No, you cannot with current release.
You can use Apache style configuration with comment lines under the general page.
