which value should I adjust to increase the time-out value?

the value there is already 120
the user is reporting that he got the error in much shorter than 120 seconds.

maybe caused by something else. check max_post_size and file_upload_size, etc in php.ini

If it can be reliably reproduced, we will take closer look.

Id try restarting Apache - maybe its possible somehow that timezone information got changed in the middle of it starting up and some threads have the old one and some the new one?

timezone does not matter, as LSWS does not use local time internally, timezone has no effect. It only affects logging.
