Have you checked the wiki to verify that you set it up correctly? If so, please share your Report number so the support team can take a look.
(Navigate to LiteSpeed Cache > Report, press the Send to LiteSpeed button, then you should see "Report Number" there.)
Yes it was setup according to the wiki, however I've dropped MaxCDN due to them replying they would not provide support since I was using an unapproved cache plugin. I will be looking into other options for this.
