Litemage cache header not appearing

I have tried troubleshooting with the wiki but none works.

I have make sure I have done all this

1) Download extension, install <-- Done
2) Add into .htaccess <--- Done
<IfModule LiteSpeed>
LiteMage on
</IfModule>
3) Refresh cache <--- Done
4) Give permission <--- Done

Anything did I missed out?

HI nicky,
Is it a standalone web server or with any control panel?
May I ask how you did to setup for cache root folder for both server level and virtual host level?
