    soft/hard limit is ok. APC is not working well with php 5.4 or 5.5. this is APC's issue. you've to use XCache or OPcache (http://www.php.net/manual/en/book.opcache.php) if you use php 5.4. or you use php 5.3, which works well with apc.

    can you force-reinstall 4.2.9 to ensure latest version, and try again ? "L" mean last rewrite rule. this will disable rest rewrite rules. so better to remove "L".
