Mean while we have set a scheduled task to restart lsws everyday in low-traffic time to reset server + APC state, will see if the problem occurs again...
That's what I would suggest. fixing APC bug is beyond what we can help.
strace the running lsphp5 process to find out what happened if it still happens.
