it should be OK. The root cause may be somewhere else. I installed fail2ban on 2 of our servers some time back. exactly same time stamp as yours.

can you post 1~3 lines of access log which still access the image files from myspace etc? so we can analyze why the rewrite rule not working?

when lshttpd reach 100%, can you capture a few seconds what it's doing? #strace -p <pid-of-lshttpd> also, output of "pstree -p", lshttpd subtree may give some clue.

it may be normal. for example a website mainly for file upload, the file upload time is long and thus "Req in Processing" keeps increasing. and "Req/Sec" is new requests.
